Date of last meaningful update (YYYY-MM-DD). Useful for prioritizing review.

Alternate contact or team responsible when the primary is unavailable.
Clear, testable conditions the new owner must meet to accept responsibility (e.g., run X task end-to-end, verify Y metric within range).
Concrete steps to verify acceptance criteria (commands, checks, sample transactions, data to inspect).
Indicate whether live shadowing, recorded walkthrough, or knowledge-transfer session is planned.
Dates, participants, duration, and what will be demonstrated. Leave blank if not scheduled.
Where artifacts live (links, folder paths, ticket IDs) and who has access. Include steps to request access if needed.
Any remaining tasks to complete the handoff, assigned owners, and target dates.
