This short diagnostic helps you quickly surface common weaknesses that keep improvement work from landing: missing documentation, unclear ownership, weak measures, infrequent defect review, and recurring rework loops. Answer each item, use the scoring guide to estimate a simple risk score, add short observations, then save the result to your team record.

Scoring guide: For each problematic answer add 1 point: undocumented process, unclear ownership, missing both outcome and quality metrics (or only one), defects reviewed rarely/never, recurring rework present. Total score 0–1 = Low priority; 2–3 = Moderate priority; 4–5 = High priority. Recommended next actions below are matched to these bands.

Include SOPs, step lists, flowcharts, or where work instructions are stored.
Owner = the person accountable for the step's outcome, not only the performer.
Outcome metrics measure value delivered (e.g., on-time rate); quality metrics measure defects, error rate, or rework. Missing either is a risk to targeted improvement.
Actioned means issues are assigned, tracked, and have follow-up.
Recurring rework is a strong signal that root causes are not being addressed.
