Use this one-page journal to record experiments so learning survives staff changes and teams can reuse what works. Keep each entry focused: state a clear hypothesis, describe your design and metrics, record results, and decide a concrete next step.

Short name to identify this experiment (e.g., 'Reduce checkout time').
Entry date (YYYY-MM-DD or your local format).
Person responsible for running and following up on the experiment. Include role or team if helpful.
What you expect to happen and why. Make it specific and testable (If X then Y because ...).
Describe the experiment design, treatment and control, sample or segment, duration, and important constraints.
List primary and secondary metrics and provide baseline values where known (e.g., conversion 3.2%).
Summarize key results with numbers. Note important caveats (sample size, seasonality, data quality).
How confident are you in the result? (1 = low, 5 = high)
