Use this checklist to make prompts safer, more predictable, and easier to review. Answer each item, add notes or examples, record owners and test results, and save the checklist so your team can track progress across versions. This is a practical operating checklist — adapt fields to match your policies and risk profile.

Have you explicitly documented what the prompt is allowed to return and what it must never return?
Provide short examples or link to the policy that defines allowed/disallowed outputs.
Have you listed plausible failure modes (e.g., hallucination, incomplete answer, unsafe advice, private data leakage)?
Example: hallucinations when source data is missing; overconfident answers for ambiguous prompts.
Select the types of outputs or cases that must be routed to a person for review.
Name or role responsible for human review (e.g., Content Moderator, Clinical Lead).
How many representative test prompts / inputs did you create to validate behavior?
How many test cases produced acceptable outputs according to your criteria?
Estimate how often the model produced unsupported or fabricated facts during testing (0 = none, 5 = frequent).

Did you explicitly test prompts that could touch sensitive topics (medical, legal, financial, PII, protected classes)?
Record sample outputs and the mitigation applied (e.g., rephrase, escalate, remove).
Select the risk categories you tested and validated.
Choose the current level of runtime monitoring for the workflow.
Describe the rollout approach (dark-launch, internal-only, pilot with human-in-the-loop, full production) and stop-gates.
Has the designated approver signed off on production use given test results and mitigations?
Name or role of the person who approved production use.
When should this checklist be re-run or the prompt be re-evaluated? Use a date or cadence (e.g., 90 days).
