This is a quick, non‑punitive near‑miss report to capture what happened, stop hazards, and spread learning. You may report anonymously. Focus on facts, immediate fixes, and who will own follow‑up. Use plain language so others can act.

Where did it happen? Be specific: line, cell, machine ID, bay, or area.
Select the most relevant team or unit.
Choose Yes to omit contact info; choose No to allow follow‑up questions.

What happened? Describe the sequence of events and conditions. Be factual and concise.
What did you or others do right away to make the situation safer?
Describe the possible harm, downtime, quality loss, or cost if the hazard had not been stopped.
Rate the potential severity if the near miss had resulted in an incident (1 = minor, 5 = severe).
