Purpose
This card helps shift leaders and supervisors do quick, non-punitive safety observations, coach the behavior in the moment, and capture follow-up actions. Aim for brief, frequent checks rather than infrequent audits. The goal is to reduce harm, surface recurring hazards, and build simple everyday safety habits.
How to use
- Do two brief observations per week per shift leader (or adapt frequency to your context).
- Observe for 2–5 minutes, note the behavior, coach immediately when appropriate, and record the observation so trends can be tracked.
- Use the 'recurring' flag and risk severity to escalate to the near-miss / corrective action system when necessary.
Tips for constructive observations
Be specific about what you saw, avoid blaming language, focus on conditions and choices rather than personality, and offer a clear, simple improvement or support.
Examples
Safe behaviors: wearing required PPE properly; using a lockout procedure; following machine guarding rules; keeping walkways clear.
Unsafe behaviors: bypassing a guard; using a phone while driving a company vehicle; not using a required harness; stacking materials unsafely.
