For dates use YYYY-MM-DD. For recurring audits, set Frequency. If you need to record multiple schedule items, submit one per item or paste a CSV into the 'Bulk schedule CSV' field.

Describe the nonconformance observed. Be specific about what was seen or measured.
Records, photos, timestamps, part numbers — describe where the evidence is located.
Immediate actions taken to limit impact.
Person or role assigned to lead root-cause analysis.
Person or role responsible for corrective action and completion.
Target date for CAPA completion.
How the fix will be verified and what evidence will close the CAPA.
Any other relevant context, reference numbers, or tags.
Name or user id of the person submitting the entry.
