Use this form to record a complete measurement system entry. Each saved entry should represent one measurement method or instrument group (for example, 'Datalogger + Thermocouple for Furnace #1'). The form collects the key elements you need to make measurements trustworthy: what you measure, which instruments and specs, calibration cadence and records, sampling and handling, data capture format, uncertainty, QA checks, and actions when things go wrong. Store certificates in your LIMS or shared records system and reference them below.

Short name describing this measurement system (e.g., 'pH probes – Reactor A').
What you measure and why. Describe decision thresholds or endpoints this measurement supports.
List instruments (make/model, serial number), measurement range, resolution, location. One instrument per line. Example: 'Model X, SN1234, 0–14 pH, 0.01 pH, Lab bench'.
Interval and rationale (e.g., 'monthly' or 'before each campaign' or 'per SOP XYZ').
YYYY-MM-DD or reference to the calibration record ID.
Where certificates and calibration records are stored (LIMS path, shared drive, paper folder) and any record IDs.
Name and role responsible for owning calibration and measurement quality.
Approach used to collect samples for this measurement.
Typical number of samples or replicates per measurement.
Describe preservation, containers, temperature, maximum holding time, chain of custody.
File formats, templates, database/table names, column headings, units, and required metadata (operator, timestamp, instrument SN).
Routine verification steps (blanks, reference standards, control charts) and pass/fail criteria. Include control limits where applicable.
Estimated uncertainty (e.g., '±0.02 pH') and short note on how it was determined (manufacturer spec, GUM estimate, inter-lab comparison).
Traceability chain to national standards or certified reference materials. List reference standard IDs or certificate numbers.
Immediate containment, investigation steps, who to notify, and disposition of affected data or samples.
How severely an incorrect measurement would affect decisions, safety, or product quality.
Preferred next calibration date or condition (e.g., 'after 1000 hours of use' or '2026-03-01').
Related SOP IDs, standards (ISO, ASTM), supporting documents or process notes.
Link or identifier for saved record in LIMS or shared drive (e.g., LIMS:CAL-2025-001). If attachments are stored elsewhere, provide the path or ID.
