Use this form to record essential metadata for each experiment so others (and future you) can find, understand, reproduce, and reuse your work. Prefer links to canonical protocol and data locations rather than scattered notebook notes. Fields marked required are minimal for traceability.

Unique identifier (use your lab or project prefix, e.g. LABX-2026-001). Required.
Short descriptive title of the experiment. Required.
Name of the PI or lead scientist.
Who to contact about protocol details or data (email or username).
Describe the hypothesis or primary question this experiment addresses.
Key objectives or measurable aims for this experiment.
Canonical protocol identifier and version. Prefer a persistent link to the protocol document.
URL or repository path to the procedure or SOP (e.g., lab wiki, protocol repository).
YYYY-MM-DD. If unknown, enter approximate date.
YYYY-MM-DD or expected end date.
Current experiment status.
Links to raw and processed datasets (repository URLs or internal storage paths). List each on a new line. Include access instructions if restricted.
Short name of the storage system or bucket (e.g., S3 bucket, LIMS project, server path).
Primary file or data format(s). Choose best match.
Links to code, notebooks, container images, or analysis pipelines. Include version or commit hash where possible.
Briefly describe runtime environment (e.g., Python 3.10, R 4.2, Docker image tag).
List important packages, libraries, and exact versions used for analysis.
Instrument make/model and key settings (e.g., centrifuge model and RPM, microscope objective).
List reagents with supplier and lot numbers that affect reproducibility.
Reference sample identifiers or accession numbers used in the experiment.
IRB, IACUC, or permit identifiers and links if applicable.
Yes if human-subject datasets require special handling.
Any safety class, containment level, or special disposal instructions.
DOIs or links to relevant papers (published or in-prep).
Short summary of main findings. Prefer concise bullet-style notes.
How the results answer the hypothesis or next-step implications.
Practical notes that help others reproduce or avoid errors.
Rate how reproducible you expect this experiment to be, with 1=low and 5=high.
