When to use each method

- A3 — use when you need a compact problem story, analysis, and implementation plan that travels to stakeholders.
- Experiment Backlog — use for testing countermeasures quickly and learning before large-scale rollout.
Facilitator checklist (quick)
- Clarify the problem statement (what, where, when, magnitude). 5 minutes.
- Choose the method based on scope (5 Whys / Fishbone / A3). 2 minutes.
- Set timebox and roles: facilitator, recorder, process expert, data owner. 1 minute.
- Gather evidence: data points, photos, logs. 5–10 minutes.
- Run analysis with team, capture hypotheses and actions. 20–40 minutes depending on scope.
- Create experiment backlog and assign the first owner. 10 minutes.
- Agree verification criteria and next review date. 2 minutes.
Facilitation script: 5 Whys (10–20 minutes)
- State the problem clearly and show evidence.
- Ask: “Why did that happen?” Record the answer as fact-based as possible.
- For each answer, ask “Why?” again — repeat until a root cause that the team can act on emerges (typically 3–6 whys).
- Validate each why with data or a quick check. If an answer is speculation, mark it as hypothesis and plan an experiment to test it.
- Convert the root cause into a specific countermeasure with owner and verification step.
Fishbone: quick guide
Draw the spine and add major categories suitable for your context (e.g., People, Process, Materials, Machines, Measurement, Environment). Brainstorm causes under each category, then circle the most plausible causes to investigate. Turn top candidates into experiments or corrective actions.
A3 template (one-page)
- Background & Problem Statement: Context, impact, why it matters.
- Current Condition: Key data and a succinct description.
- Goal/Target: Measurable outcome and timeline.
- Root Cause Analysis: Evidence-backed findings (5 Whys / fishbone summary).
- Countermeasures / Experiments: What you will try, owners, and metrics.

Experiment Backlog (template)
Use this to convert hypotheses into small, timeboxed tests.
- Title — short name

Prioritize using a simple impact vs. effort matrix: fast wins first, then high-impact experiments.
Prioritized Improvement Action template
For each approved countermeasure capture:
- Action description (clear and testable)
- Owner
- Due date
- Verification criteria (how you will know it worked)
- Dependencies and escalation triggers
Escalation guide (when to escalate)
Escalate when experiments repeatedly fail, required resources exceed team authority, or the risk/impact is larger than the team’s delegation. Provide a concise one-paragraph summary and the recommended decision (approve resources, assign cross-functional owner, or pause the solution).
How to use this toolkit in practice
- Start small: run a 30–60 minute session with 5 Whys + experiment backlog for quick wins.
- For recurring or complex issues, use Fishbone + A3 to build a one-page story and plan.
- Track experiments and actions in the backlog; review weekly until verification criteria are met.
- Store completed A3s and experiment outcomes in a team learning folder for future reference.
