Use this checklist to make operational handoffs predictable so validated ideas deliver sustained impact. Complete items, attach links to artifacts, assign owners, and submit to create a persistent record that teams can act on and track.

This form is intentionally pragmatic: it captures the minimum useful information for reliable handoff and highlights common failure modes such as missing runbooks, unclear acceptance criteria, or absent monitoring.

Person or team responsible for day-to-day operation after handoff. Include contact info or a link to the org directory.
Paste links to runbooks, diagrams, SOPs, code repos, runbook sections, and data mappings.
Clear, testable conditions that define a successful integration (examples: latency < X, error rate < Y, data completeness > Z).
Links to training materials, target audiences, delivery method, and how completion will be verified (e.g., sign-off, quiz, or attendance).
What will be monitored, which dashboards exist, alert thresholds, and who triages alerts. Include links to dashboards/alert rules.
Support hours, expected response times, escalation path, and any committed service levels.
Where to capture early improvements or defects (backlog location) and the team responsible for triage and prioritization.
Steps to revert changes, criteria for rollback, who approves, and communication plan for stakeholders and users.
Describe data feeds, schemas, ETL jobs, frequency, owners, retention policies, and any transformation logic or contractual data obligations.
List regulatory, privacy, security, or contractual requirements checked or still required. Link to security review or DPA if applicable.
YYYY-MM-DD or approximate date.
Who must sign off before and after go-live (e.g., product owner, operations lead, security). Include role and name where known.
Primary and escalation contacts, with preferred communication channels (phone, pager, Slack channel).
Links to deployment tickets, PRs, release notes, build numbers, runbook versions.
1 = Not ready, 5 = Fully ready
Any other observations, risks, or requests.
