Scoring & Use

Each item uses a 1–5 scale where 1 = Strongly disagree and 5 = Strongly agree. Calculate average scores for each domain and compare across teams or over time. Rough guidance: average ≥ 4 = strength; 3–4 = mixed/targeted improvement; < 3 = priority area for action. Combine quantitative findings with qualitative comments and follow-up conversations before major interventions.

Sample Size Guidance

For small teams, aim for at least 20–30 responses or ~20% of the group to get a reasonable pulse. For larger organizations, use stratified sampling to ensure representation across functions, levels, and locations. Consider anonymizing responses to improve candor.

Confidentiality Note

Surveyors should decide whether to collect identifiable information. If confidentiality is a concern, prefer anonymized submissions and supplement with structured interviews or focus groups.
