Use this checklist to move an innovation, experiment, pilot, or improvement into dependable day-to-day operations. Each item helps reduce the common causes of failed handoffs: unclear ownership, missing acceptance criteria, absent monitoring, undocumented automations, and lack of training. Record owners, links, notes, and a final operations acceptance sign-off to complete the handover.

Who in operations will be the day-to-day owner after handover? Include name and role.
Use YYYY-MM-DD or a relative timeframe (e.g. '2 weeks after pilot')
Runbook should include step-by-step operations, troubleshooting, recovery, and links to artifacts.
Paste links or a short pointer (team drive path, repo, ticket) to the canonical runbook and supporting documents.
Include L1/L2 contacts, vendor support, and escalation SLAs.
List phone, email, ticket queue, Slack channel, or other contact points.
Define what 'acceptable steady-state' looks like and measurable success criteria.
List the criteria and thresholds (e.g. error rate < 1%, latency < 200ms, defect rate < 0.5%).
Which metrics are monitored, where dashboards live, and which alerts trigger action.
Provide concrete metric names, dashboard links, and escalation steps when alerts fire.
Confirm APIs, scheduled jobs, ETL processes, connectors, and their owners.
List systems, topics/queues, credentials needs, and where logs are stored.
Documents, videos, quick-reference guides, and on-the-job exercises for operators.
Pick the current state of operator training
Links to slides, recordings, LMS modules, checklists, or learning objectives.
Who does L1/L2, software fixes, releases, and vendor management after handover?
Include go/no-go criteria, communication plan, and backout triggers.

Typical cadence: 2 weeks, 30 days, and 90 days after go-live.
List unresolved issues that could prevent a safe handover. Include owner and target resolution date for each.
Architecture diagrams, security approvals, compliance evidence, tickets, or vendor contracts.
Formal acknowledgement from the operations owner that they accept responsibility for steady-state operation.
