Welcome. Use this concise gate review form to surface the evidence, compare outcomes to success criteria, assess risk, and decide the next action for a discovery or early-stage innovation project. This template is optimized for a 20‑minute decision meeting: capture concise evidence, recommend an action, and record clear owners and next steps so the work can continue with clarity.

Tip: Focus on the most meaningful evidence (metrics, experiments, customer signals), be explicit about what success would look like, and name the person who owns the decision and follow-up.

Concise name for the initiative.
YYYY-MM-DD or local date format.
Name and role of the person presenting the evidence.
Choose the review stage (tailor options to your portfolio).
Summarize key evidence presented: experiments run, data points, prototypes, customer feedback, or pilot outcomes. Include references to attachments or dashboards if available.
List the predefined success criteria and state whether each was met, partially met, or unmet. Be specific (metric, result, target).
Provide the most important metrics (value, unit, baseline, target). Example: conversion 3.2% (target 5%).

List the main technical, customer, market, regulatory, or operational risks that could change the outcome and how likely/impactful they are.
Choose the recommended direction based on evidence and risk appetite.
Estimate time, budget, and key roles required for the recommended action (include magnitude and duration).

Person accountable for the decision and follow-up.
Record names, roles, and (if available) platform identities of the meeting approvers. The system will also store the submitting user identity when available.
