This interactive bundle helps perioperative teams standardize evidence‑informed practices that reduce procedure‑related infections, capture essential compliance data, and support local surveillance and improvement. Use the checklist items during the OR start and sign‑out workflows and save each submission so teams can monitor compliance and trends.

How to use

  • Complete the pre-op section in the preoperative area (or during pre-op timeout).
  • Complete intra-op sections by the circulating nurse or designated observer.
  • Complete the post-op teaching and surveillance sections before patient discharge and again at first wound check when applicable.

Suggested targets (examples to adapt locally): antibiotic administered within 60 minutes of incision ≥95%; overall bundle adherence ≥90%; monitor 30‑day SSI rate against baseline.

Mal hungers: avoid adding checkbox busywork—use this form to gather only useful, auditable data and link it to improvement work. This tool does not replace clinical judgment or infection prevention programs.
