Sterile Processing Turnaround & Tracking Checklist (Interactive)

An interactive checklist and tracking entry form to record instrument tray lifecycle events, ensure sterilization monitoring and traceability, capture delays with escalation, and feed monthly performance metrics (turnaround time, failures, escalations).

Sterile Processing Turnaround & Tracking Entry

Purpose

This interactive entry captures each tray/set lifecycle from receipt through return to the OR. Use it to ensure required checks are completed, to record sterilization monitoring results, to document and escalate delays, and to create reliable data for monthly performance review.

Key definitions

  • Turnaround time: time from receipt of contaminated tray (or end of procedure) to returned sterile tray available for case use. Record minutes or timestamp values consistently.
  • Critical monitors: Biological Indicator (BI), Chemical Indicator (CI), and Process Challenge Device (PCD) where used. A BI failure requires quarantine, notification, investigation, and reprocessing.
  • Escalation: when a tray is projected to miss the target turnaround threshold or a sterilization monitor fails. Follow local escalation contacts immediately.

Suggested thresholds (example)

  • Standard turnaround target: 90 minutes (adjust to local baseline)
  • Escalation trigger: projected turnaround > 120 minutes or BI failure

Record times using the same timezone and format (YYYY-MM-DD HH:MM). If your site uses barcodes/scan data, paste or scan the ID into the Tray / Set ID field. Submissions are stored for aggregated reporting.

Enter the unique tray or set identifier. Use barcode/scan when available.
Optional—link this tray to an OR case or procedure identifier.
When the contaminated tray arrived in SPD or was picked up. Use a consistent timestamp format.
Time when cleaning/washer-disinfector verification was completed and documented.
Person who verified cleaning.
When the load entered the sterilizer and the cycle began.
When the sterilizer cycle completed.
Equipment cycle identifier or load number from sterilizer printout.
Select the sterilization method used for this tray.
Record BI result when applicable. If BI pending, mark Pending and follow up.
Record CI result for the load.
Indicates whether a PCD was included and monitored.
Visual inspection of instruments and packaging.
Check instrument function where applicable.
Confirm packaging intact and indicator tape/peel-pack condition.
When the sterile tray was returned or marked available.
Minutes from receiving_time to returned_to_or_time. If you calculate externally, enter the number here.
Select Yes if the tray missed the target or required escalation.
Choose the primary cause of the delay.
Describe what happened and immediate corrective actions taken.
Indicate whether escalation contact(s) were notified per protocol.
Who was notified (names, roles, pager/phone). Include time if known.
Person completing this form.
Include disposition, investigation actions, or required audits.
