Sterile Processing Turnaround & Tracking Checklist (Interactive)
An interactive checklist and tracking entry form to record instrument tray lifecycle events, ensure sterilization monitoring and traceability, capture delays with escalation, and feed monthly performance metrics (turnaround time, failures, escalations).
Sterile Processing Turnaround & Tracking Entry
Purpose
This interactive entry captures each tray/set lifecycle from receipt through return to the OR. Use it to ensure required checks are completed, to record sterilization monitoring results, to document and escalate delays, and to create reliable data for monthly performance review.
Key definitions
- Turnaround time: time from receipt of contaminated tray (or end of procedure) to returned sterile tray available for case use. Record minutes or timestamp values consistently.
- Critical monitors: Biological Indicator (BI), Chemical Indicator (CI), and Process Challenge Device (PCD) where used. A BI failure requires quarantine, notification, investigation, and reprocessing.
- Escalation: when a tray is projected to miss the target turnaround threshold or a sterilization monitor fails. Follow local escalation contacts immediately.
Suggested thresholds (example)
- Standard turnaround target: 90 minutes (adjust to local baseline)
- Escalation trigger: projected turnaround > 120 minutes or BI failure
Record times using the same timezone and format (YYYY-MM-DD HH:MM). If your site uses barcodes/scan data, paste or scan the ID into the Tray / Set ID field. Submissions are stored for aggregated reporting.
