Use this short interactive huddle form to run consistent 10–15 minute safety huddles that surface immediate risks, assign owners, and capture follow-up actions. The form records a compact data snapshot (current value, 24h change, 7d trend) for core safety metrics, limits the number of escalation items, and stores owners and due dates so follow-up is visible across shifts.

Suggested cadence: shift-start plus a mid-shift check-in for high-acuity units. Keep the huddle to the timebox: update numbers quickly, limit escalations to three items, and capture clear owners.
