2. Medication reconciliation on admission (chart review)
Measure — Numerator: patients with a completed medication reconciliation within 24 hours of admission. Denominator: all admitted patients eligible for reconciliation. Exclusion: transfers from other acute settings with completed reconciliation documented.
Sampling suggestion — For low-volume units sample most admissions; for high-volume, a rolling sample of 30 records/month stratified by weekday can show trends.
3. Sepsis bundle compliance (process measure)
