Use this form to capture incoming safety signals (incidents, near-misses, data trends, complaints) and apply a consistent prioritization approach. The form collects intake data, scores impact and detectability, guides a decision using a simple rubric, and records minimum next steps and owners. Run this with your safety champions and analytics owner during weekly triage.

Quick scoring guidance: Use the scales below (1 low — 5 high). A high-priority signal usually has high harm severity or high frequency. If Severity >= 4 or Frequency >= 4, consider Investigate now. Use Detectability to raise priority when signals are hard to detect (low detectability increases urgency). Suggested mapping: if (Severity + Frequency) >= 7 -> Investigate now; if 5–6 -> Monitor or Quick fix depending on effort; <=4 -> Defer or Monitor. Adapt the rubric locally and record your decision.
