Describe where and when this task begins and ends, patient population, settings, and any exclusions (what this checklist does not cover).
Who is expected to perform this task and what training, certification, supervision, or competencies are required?
List each step on its own line. Prefix the role in square brackets, e.g., [RN] Verify order. For each step, include the expected outcome and estimated time (optional). Example line: '[RN] Verify patient identity and allergy status — ~2 min.'
If helpful, list approximate minutes for individual steps or a single total estimate. Useful for workload planning and onboarding.
Highlight critical verification points and 'must not miss' checks and how to confirm them (e.g., double-checks, independent verification, time-outs).
List exact items (including sizes, concentrations, disposables, software screens, or device settings). Include alternatives if applicable.
Describe common variants (patient types, equipment differences) and clear criteria for escalation: when to stop, who to notify, and immediate next steps.
Provide links to images, diagrams, annotated screenshots, or note which steps need accompanying visuals. Store images in the approved repository and paste links here.
List typical mistakes or failure modes and brief mitigations or detection strategies (for example: 'Wrong medication concentration — add independent concentration check').
Identify the owner and required approvers by role and how sign-off will be recorded (electronic approval, signature, or committee).
Choose how often the DWI should be formally reviewed or whether it is updated only when processes change.
Record version number, date, and a short summary of the change when this checklist is updated.
Approximate time to complete the entire task — helpful for staffing and training estimates.
