Purpose

Quickly capture when work is happening, when it stops, and why—using operator-friendly categories. Save structured observations for later aggregation so teams can find the biggest loss categories and prioritize practical experiments.

How to use this form

1. Fill the shift header (date, line, shift).
2. Log events in chronological order. For each event record the start and end time, select a category, enter minutes (or let the form calculate minutes from start/end), and add a short note with facts (what stopped, who, tool, material).
3. If an event spans a planned break or scheduled downtime, mark it as Planned Downtime. Use Unplanned Downtime for unexpected stops.
4. When finished, submit. Data is stored for later aggregation and analysis.

Operator-friendly categories

Productive, Planned Downtime, Unplanned Downtime, Quality Rework, Support, Changeover/Setup, Material Shortage, Maintenance, Training/Meeting, Safety, Other.

Quick analysis notes (for team leads)

After you collect several shifts, total minutes per category, compute percent of scheduled time that was productive, and rank categories by total lost minutes. Focus experiments on the top 20–30% of loss categories that represent 70–80% of lost time. Start with rapid experiments (tooling, material staging, quick SOP tweaks, changeover steps) and follow with short audits and PDCA cycles.
