A single-page interactive coaching card supervisors can use on Gemba to capture respectful, operator-centered observations, create short experiments, assign owners, and track follow-up so availability and performance gains are documented and measurable.
Use during Gemba: Keep it short, factual, and respectful. Capture what you observed, what you did to contain the problem, a quick root-cause prompt to focus an experiment, the experiment you want to try, what to measure, who owns the follow-up, and when you will check back. This form saves the coaching note so teams can track experiments and measurable improvements. Avoid blaming language — focus on conditions and fixes.
