When to use LOTO
- Anytime someone must place a body part where they could be exposed to hazardous energy (mechanical, electrical, hydraulic, pneumatic, thermal, chemical, stored energy).
- Before troubleshooting, inspecting, cleaning, adjusting, or performing maintenance that could unexpectedly start a machine or release energy.
- When lockable isolation is reasonably possible and will remove the risk—if not, use alternative controls agreed with your EHS team.
Quick operator sequence (plain-language)
- Plan & notify — Identify all energy sources, required tools, and persons to notify. Tell affected coworkers and supervisors what you will do and why.
- Prepare — Gather tags, locks, hasps, and required PPE. Verify you have the right LOTO devices for this machine.
- Shutdown — Turn off the machine using normal stop controls.
- Isolate — Physically isolate each energy source (switches, breakers, valves). Close and secure valves when needed.
- Lock & tag — Apply your personal lock and a durable tag showing your name, date, and contact. Use additional locks if required by the procedure (multi-person hasps, multiple locks).
- Release stored energy — Bleed, vent, block, or restrain springs, flywheels, hydraulics, or other stored energy sources.
- Verify isolation — Try to start the machine with normal start controls. Use a non-contact tester or appropriate method to confirm zero energy if electrical. Only then begin work.
- Perform work — Carry out maintenance, adjustment, or inspection.
- Restore — When work is complete, ensure tools and guards are replaced. Remove lock and tag only if you are the person who applied them. Re-energize following the agreed procedure and notify affected people before restarting.
Verification checks (must-do before work)
- Confirm all energy sources identified on the isolation plan have been secured.
- Confirm personal lock is in place and prevents operation.
- Confirm stored energy is released or blocked.
- Attempt to restart the machine using normal controls (without removing locks). Confirm machine will not start.
Handover & transfer of lockout (practical rules)
Transfers are common during shift changes. Use a formal, recorded transfer process:
- Incoming and outgoing operators meet at the machine and review the LOTO status and work done.
- Outgoing operator explains why locks remain in place and lists remaining tasks and hazards.
- Incoming operator places their own lock and signs transfer log before outgoing lock is removed, or both maintain locks until the incoming owner applies theirs (site-specific practice).
- Document the transfer in the shift log or LOTO transfer form and notify the supervisor.
Required PPE & tools
- Minimum: safety glasses, hearing protection if needed, cut-resistant gloves where sharp edges exist.
- Electrical work: arc-rated PPE as required by the electrical safe work procedure.
- LOTO devices: uniquely keyed personal padlock, durable tags, hasps, valve lockouts, circuit lockouts, and appropriate identification labels.
Common operator mistakes to avoid
- Assuming a machine is de-energized without verifying.
- Using removable or unreliable tags or locks that can be bypassed.
- Removing another person's lock—never do this unless a formal removal process is followed.
- Failing to identify hidden or secondary energy sources (e.g., gravity, springs, residual pressure).

- Have I identified all energy sources and isolation points? (yes / no)
- Have I notified affected personnel? (yes / no)
- Have I shut down and isolated each energy source? (yes / no)
- Have I applied my lock and tag and recorded my name/date? (yes / no)

What to do if you find someone else’s lock
Never remove another person’s lock. Follow your site's substitution/removal policy which normally requires supervisory verification and documented attempts to contact the lock owner. If emergency removal is necessary to prevent harm, involve your supervisor and EHS and fully document the action afterward.
When LOTO might not be required
Some minor tasks that do not expose workers to hazardous energy can be done with alternative controls (e.g., point-of-operation guards, safe work permits). Consult your EHS team and site procedures—never make this decision alone.
