What was done to stop further loss? Who was involved?
Keep asking 'why' until you reach a meaningful root cause. If needed, run additional whys.

State the best explanation supported by evidence.
List specific, testable actions to eliminate the root cause.
Name of the person responsible for implementing/testing.
If testing a countermeasure: 'If we do X, then Y will improve by Z'.
