How to use

Root Cause Analysis needed when escapes, recurring defects, safety risk, or major nonconformance occur
Escalate to Engineering or Customer Quality when containment is insufficient or product is nonconforming
Record actions taken or assigned to stop defect production
Actions to complete after shift, including timelines and owners
Key points for the next shift leader
