Use this form to plan, run, and record a focused, time-boxed continuous improvement experiment. Keep tests small, measurable, and reversible. Saved submissions help teams review results, learn faster, and standardize what works.

Short descriptive title (include area/process) and an owner name or role.
Who will run the experiment and be accountable for results.
Describe the specific problem, its impact, and who or what is affected. Be concrete.
If we do X (change), then Y (measurable outcome) will improve by Z. Include how you expect the system to change.
Optional: e.g., 'Reduce cycle time by 20%' or 'reduce defects by 3 per 1000'.
What metric will you measure? Record the current baseline value and how it was measured.
How will success be measured? Include data sources, sampling method, frequency, and acceptable variation.
Step-by-step actions, who does what, and what controls ensure a fair test. Keep steps simple and repeatable.
How long will the experiment run? Shorter is usually better—days rather than months.
People, tools, materials, budget, shift coverage, or support needed to run the test.
How will you return to the previous state if the experiment causes harm or fails? Note safety, quality, or customer impacts.
Identify key risks and immediate mitigation steps. Focus on what could cause customer, safety, quality, or downtime issues.
When and how often data will be recorded during the experiment (e.g., daily shift summary, hourly samples).
Record the measured outcomes after the experiment ends. Include raw numbers and any notes about context.
Select the outcome based on your success criteria and measured data.
If the experiment succeeds, list steps to standardize the change: documentation, training, role ownership, and monitoring.
What worked, what didn't, surprises, and recommended next steps or follow-up experiments.
Choose yes to notify or present results at the next huddle or CI review.
