15‑Minute Huddle Agenda
- 0-3 min — Quick safety check and shift highlights
- 3-7 min — KPI snapshot (OEE, downtime, scrap/quality)
- 7-12 min — Issues triage and immediate containment
- 12-15 min — Assign owners, confirm next steps & escalation triggers
Roles: Facilitator (keeps time and focus), Scribe (records issues, owners, due dates), Escalation Owner (accepts escalations beyond the shift). Start on time. Keep each issue to a 1-2 minute containment plan. Only escalate when triggers are met. Adapt thresholds to your plant.
Suggested Escalation Triggers (examples)
- Unplanned downtime > 10 minutes
- OEE drop > 8 percentage points vs previous shift
- Scrap or defect rate increase > 2 percentage points
- Repeat problem (same issue in 2 of last 3 shifts)

How to use this form
Enter the KPI snapshot, then record up to three top issues. The scribe must assign an owner and a follow-up due date. Use the Huddle Log to review open actions at the next huddle. Keep entries brief and focused: what happened, immediate containment, owner, and due date.
