This is a simple, non-punitive near-miss report. Capturing near-misses helps us reduce hazards before incidents occur. Reports go through a quick triage and, when appropriate, a small control or experiment is assigned. We aim to respond with feedback to the originator within 5 business days.

What was done right away to make the area safe (stopped machine, isolated hazard, etc.)?
Estimate the likely harm or damage if the near-miss became an incident.
Based on what you observed (environment, frequency, condition).
A short, practical action that could reduce the risk (temporary or long-term). Include who might implement it if you can.
Volunteers speed improvements and help verify effectiveness.
