Use this interactive template to capture site- or enterprise-level decisions that make continuous improvement repeatable, measurable, and locally owned. Fill the fields below to create a one-page roadmap you can copy, share, and adapt to other sites.

Identify local sponsors who will unblock resources, remove barriers, and attend cadence reviews.

Define rules for when practices become standard work, how to integrate feedback, who approves local adaptations, and how the playbook evolves.
Select common pitfalls and use the mitigation field to record countermeasures.
Describe specific actions, owners, and timelines to mitigate the selected risks.
