This non-punitive near-miss form helps capture what happened, what stopped it from becoming an incident, and a safe test to reduce the hazard. Fill what you know — brief, factual entries are fine. If you prefer to stay anonymous, leave the reporter fields blank.

When did the near miss occur? Use local date/time (e.g., 2026-08-11 14:30).
Be specific enough to find the spot later (e.g., Line 2, Press A, Bay 5).
Briefly describe what happened in order — what led up to it, what occurred, and how it stopped from becoming an incident.
List conditions or actions that helped create the near miss.
What did you or others do to contain or stop the hazard right away?
Estimate the worst credible outcome if the near miss had resulted in actual harm.
List one or more plausible explanations (system, training, maintenance, design, human factors). These are working hypotheses to test, not final blame.
Describe a safe, quick test or temporary control to reduce the hazard (who will do it, how, and how long it will run). Keep it simple and verifiable.
Who will run or own the experiment? If unknown, enter a role (e.g., Line Supervisor).
When will the experiment be checked? Use yyyy-mm-dd. Choose a near-term date (days to a few weeks).
To be filled after the verification date. Leave as Pending for initial submission.
What should change in procedures, training, materials, checks, or design to prevent recurrence?
Select Yes if you informed a responsible person immediately.
Leave blank to report anonymously. Providing a contact helps verification and learning.
Optional. Only used for follow-up if needed.
Add short tags to help classify this report (e.g., pinch-point, conveyor, lockout, fatigue).
