Ethics & Bias Monitoring Checklist

Use this checklist during development, model release, or periodic audits to detect and manage fairness and ethics risks. Complete the sections below to capture your sampling plan, chosen bias metrics, mitigation actions, stakeholder engagement, transparency disclosures, monitoring cadence, and owner. Save a record each time you review so you can track changes over time and report to stakeholders.

How to use: answer the questions honestly; provide links or brief notes where helpful; attach model evaluation reports in your project repository and reference them in the 'Notes' field. This tool records responses for audits and follow-up.

Is there a documented sampling plan describing population coverage, strata, sample sizes, and sampling frequency?
Give a concise summary or provide a link to the full document (repo link or storage location). If no plan exists, note next steps.
Record numeric targets or rules (for example: minimum 200 examples per subgroup).
Select the metrics you actively monitor. Pick the ones that match your use case and document thresholds below.
Specify numeric thresholds or rules that trigger investigation or remediation (e.g., FPR difference > 0.05).
Name scripts, evaluation notebooks, or tools (e.g., fairness toolkit) and their repo locations. Include test cadence if automated.
Select mitigation approaches that are implemented or available to use.
Describe how labeling issues are tracked, who can request relabels, and the process for implementation and verification.
Identify which stakeholders have been consulted or will be informed during monitoring and remediation.
Which disclosures or explainability artifacts are ready to share with users or auditors?
How often are fairness checks run and reviewed? Choose the cadence that matches risk and change velocity.
Name and contact (email) of the person or role responsible for this checklist and next steps.
Record the date of this review. Use ISO format for easier downstream parsing.
Capture immediate action items (investigate metric X), remediation plans, or follow-up review dates.
Add links to evaluation reports, dashboards, issue tickets, or other evidence used during the review.
